Opt in to receive your 1095-C tax form digitally

UT Health San Antonio employees:

UT Health San Antonio has partnered with Equifax as our vendor to provide digital versions of your 1095-C tax form. The 1095-C form provides tax information about the health care benefits you receive as an employee including which months you were offered coverage and the employee required contribution for self-only coverage.

Opt in to receive your 1095-C tax form digitally

To receive your 1095-C digitally, you must consent to receive online forms on the Equifax mytaxform.com website. Find tutorials below on how to log in and opt in to receive digital forms.
If you do not opt in to receive your 1095-C form digitally, it will be mailed to your current home address on file with UT Health San Antonio. Forms for the 2021 tax year will be distributed by Jan. 31, 2022.

Using the online system offers convenience including online access to your 1095-C in real time without waiting for a mailed copy and getting a reissue of your 1095-C anytime you want.

Update your address

If you choose to receive your W-2 or 1095-C tax forms by mail, please make sure your address is up to date before tax season. To confirm your mailing address is correct or update it, follow the Instructions to access Employee Self-Service.

Receive your W-2 digitally

If you want to access your W-2 tax form digitally, opt in to do so on the UT Health San Antonio Payroll and Compensation website.
- Click on W-2/W-2c Consent to opt in to receive your W-2 digitally.
If you do not opt in to receive your W-2 digitally, you will be mailed a copy of your W-2 to your current address on file by Jan. 31.
